Payment is due at the time of service. A deposit may be required for scheduling. Cards are accepted, including HSA/FSA.

Lab orders incur a lab review fee so that your results can be reviewed in preparation for your follow up visit. See the Financial Policies upon scheduling for more details.

Insurance

I am not contracted or “In-Network” with any insurance carriers. While payment is due at the time of service, you may be eligible for reimbursement through your health coverage. Coverage is dependent upon your plan. I will provide you with a Superbill form to assist in your reimbursement submission.
